29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:      a) South side of structure, west of the rear entry; On or about 9/15/11 and at times prior thereto, a stand of tubular welded frame scaffold base plates were setting directly on the ground without mud sills having been installed prior to employees using the scaffold for installing stucco.    b) South side of structure, east of the rear entry; On or about 9/15/11 and at times prior thereto, a stand of tubular welded frame scaffold base plates were setting directly on the ground without mud sills having been installed prior to employees using the scaffold for installing stucco.    Note: Abatement certification is NOT required for this item.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a) South side of structure, west of the rear entry; On or about 9/15/11 and at times prior thereto, a stand of tubular welded frame scaffold did not have guard rails installed across the ends of the second level prior to employees using the scaffold for installing stucco.    b) South side of structure, east of the rear entry; On or about 9/15/11 and at times prior thereto, a stand of tubular welded frame scaffold did not have guard rails installed across the ends of the second level prior to employees using the scaffold for installing stucco. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    a) South side of structure, roof area above rear entry; On or about 9/15/11 and at times prior thereto, two employees were installing stucco on the gable while standing on a sloped roof without a means of fall protection.
