29 CFR 1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury. a) Computer room - On or about August 29, 2001 and at times prior thereto, the fire extinguisher is located behind a door that remains open during the workday causing a delay to access in case of emergency.

29 CFR 1910.165(e): The employer did not ensure the manually operated actuation devices for use in conjunction with employee alarms were unobstructed, conspicuous and readily accessible. a) Computer (IT) Room - On or about August 29, 2011 and at times prior thereto, the manually operated emergency fire alarm pull box was located where the room door obstructed its view and access at most times. The entry door to the room remains open for most of the working day and the pull box is located on the wall where the door hinges open preventing it from being conspicuous or readily accessible in case of emergency.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, unless located and arranged so the purpose is evident. a) Electrical room/closet - On or about August 29, 2011 and at times prior thereto, the electrical circuit breaker panels AA, AB, AC and HA supplying power to the facility were not labeled to indicate the exact area or equipment the breaker supplied power to. The entries were general such as, but not limited to, Recpts area and then quote marks below; Power pole with quote marks on breakers below; Computer with quote marks below on breakers, Copy room with quote marks below, Lighting with quote marks below, and Space with quote marks below for other breakers. When a breaker needs to be switched off for some reason including emergencies it is not possible to determine exactly which breaker needs to be switched off with the current breaker markings.

29 CFR 1904.29(b)(1): You must enter information about your business at the top of the OSHA 300 Log, a description for each recordable injury or illness was not described. (a). Bureau of Land Management - On or about August 29, 2011 and at times prior there to, the 2007 OSHA 300 entry associated with case no 2007-0501018 did not contain a description of the injury in the detail required by standard and indicated on the OSHA 300 log form. The entry for description of injury/illness stated single leg without stating which leg or area of leg, type of injury or how it occurred.
