29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: GJT Investors LLC, dba Aspen Ridge Alzheimers Special Care Center, at 622 28 1/4 Road, Grand Junction, CO: On 9/15/11, GJT Investors LLC did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from liquid chemicals, acids or caustic liquids. Dietary workers used Ecolab Inc Oasis 144 (containing alkyldiemethylbenzyl ammonium chloride) to clean surfaces in the kitchen. The employer did not ensure that employees wore eye protection when using this liquid chemical. This condition exposed employees to a chemical hazard. GJT Investors LLC, dba Aspen Ridge Alzheimers Special Care Center, at 622 28 1/4 Road, Grand Junction, CO: On 9/15/11, GJT Investors LLC did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from liquid chemicals, acids or caustic liquids. Maintenance staff changed out containers of laundry chemicals including, Ecolab Inc Laundri Destainer (containing 8.5% sodium hypochlorite), Ecolab Inc Neutralizer Concentrated Laundry Sour, and Ecolab Inc Surlite Liquid Laundry Detergent (containing 25% sodium hydoxide). The employer did not ensure that employees wore eye protection when using these corrosive chemicals. This condition exposed employees to a chemical burn hazard.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: GJT Investors LLC, dba Aspen Ridge Alzheimers Special Care Center, at 622 28 1/4 Road, Grand Junction, CO: On 9/15/11, GJT Investors LLC did not ensure that where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were provided within the work area for immediate emergency use. Dietary workers used Ecolab Ultra Limeaway (containing 32% phosphoric acid and 3% hydrochloric acid) to clean the dishwasher and Ecolab Inc Greasestrip Plus oven cleaner (containing 7% sodium hydroxide) to clean the oven. Phosphoric acid, hydrochloric acid, and sodium hydroxide are corrosive liquids. The employer did not provide an emergency eyewash in the kitchen. This condition exposed employees to a chemical burn hazard.

The employer did not perform an annual review of its medical devices. This condition exposed employees to a bloodborne pathogen hazard.

29 CFR 1910.1030(d)(4)(iii)(A)(2)(i): During use, containers for contaminated sharps were not easily accessible to personnel or located as close as was feasible to the immediate area where sharps were used or could be reasonably anticipated to be found: GJT Investors LLC, dba Aspen Ridge Alzheimers Special Care Center, at 622 28 1/4 Road, Grand Junction, CO: On and preceding 9/15/11, GJT Investors LLC did not ensure that during use, containers for contaminated sharps were easily accessible to personnel or located as close as was feasible to the immediate area where sharps were used or could be reasonably anticipated to be found. Nurses used Humalog KwikPen insulin injectors and LifeScan OneTouch lancets when caring for residents at the long term care center. After use, the contaminated sharps were carried from the interior of resident rooms through the doors to the med cart, approximately twelve feet away, where the contaminated sharps were removed from the injectors or lancets and disposed in sharps containers. The employer did not ensure that sharps containers were located as close as feasible to where the sharps were used. This condition exposed employees to a bloodborne pathogen hazard.
