29 CFR 1910.179(k)(1)(ii):   The actuating mechanism of the limit switch was not located so that it would trip the switch, under all conditions, in sufficient time to prevent contact of the hook or hook block with any part of the trolley:  (a) In building 875, the upper limit switch was not adjusted on the Zenar Corp (S/N:  4747) to accommodate for the Caldwell Strong Bac spreader bars.  This allowed the spreader bar to hit the girder of the Zenar Corp exposing employees to struck by hazards.

29 CFR 1910.1030(f)(2)(i):   Hepatitis B vaccination was not made available after the employee had received the training required in 29 CFR 1910.1030(g)(2)(vii)(I) or within 10 working days of initial assignment to employees who had occupational exposure: to blood or other potentially infectious material.  (a)  The employer did not have a provision for the full hepatitis B vaccination series to be made available as soon as possible, but in no event later than 24 hours, to all unvaccinated first aid providers who had rendered assistance in any situation involving the presence of blood or OPIM, regardless of whether or not a specific "exposure incident" as defined by the standard, had occurred.  This exposed employees to potentially infectious material.
