29 CFR 1910.147(d)(3): All energy isolating devices that are needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy sources: (a) On or about June 6, 2014 an employee was disassembling the lower capping unit for an Arol Capper and did not block the lower capping unit in such a manner as to isolate the release of gravitational energy, exposing the employee to crushing hazards.
