29 CFR 1910.141(a)(3)(i):     Places of employment were not kept clean to the extent that the nature of the work allowed:  a) Samoa Motors Inc, mechanic shop mens shower room:  On or about 31 AUG 11, the shower room was used as storage for tires, metal wheels, engine parts and paints denying the mechanics the chance to clean up and taking shower anymore at work. This practice also dried up drainage pipes allowing sewer gas in the workplace exposing workers to a health hazard.
