29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    (a) All Around Roustabout, LLC @ 30985 WCR 64, Gill, CO 80624: On and before 9/26/11, the employer did not ensure that the floor above the parts storage area was guarded by standard railings. Employees accessed the top of the parts storage area to store various parts and vehicle-related materials. This condition exposed employees to an 8 foot fall hazard.

29 CFR 1910.334(a)(2)(i): The employer did not ensure that portable cord- and plug-connected equipment and flexible cord sets (extension cords) were visually inspected  before use on any shift for external defects and for evidence of possible internal damage:     (a) All Around Roustabout, LLC @ 30985 WCR 64, Gill, CO 80624: On and before 9/26/11, the employer did not inspect the extension cords that were used to power outdoor diesel and gasoline fuel pumps. Employees access the fuel pumps daily. The outer jacket on the extension cords were damaged thereby exposing the inner conductors. The jacket for the inner conductors, however, was not damaged. This condition exposed employees to shock hazards.

Employees access the fuel pumps daily. The outer jacket on the extension cords were damaged thereby exposing the inner conductors. The jacket for the inner conductors, however, was not damaged. Employees stored various parts and materials on top of the parts cage.     (b) All Around Roustabout, LLC @ 30985 WCR 64, Gill, CO 80624: On and before 9/26/11, the employer did not ensure that a storage system was equipped with a load rating. The employer purchased a storage system where 4, 275 gallon totes containing various vehicle-related fluids were located.

The employer purchased 5-gallon water containers are used, after exhausted, for storing other liquids such as, but not including, antifreeze and non-potable water.
