OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught-in-between hazards:     (a)All Around Roustabout, LLC, Noble Energy NCLP-AA04-66-1HN, Briggsdale, CO 80611: On and before March 19th, 2015, the employer did not ensure that an employee involved in removing and replacing two dowel pins that connect the John Deere 624J loader coupler mechanism hydraulic to a pin was protected from caught-in-between hazards. The mechanic was attempting to line up the holes to install a new dowel pin when the loader operator extended the pin and caught the mechanics fingers between the pin fastener end and the hole for the pin. The employee sustained a laceration of the right middle finger and an amputation of approximately 2 inches to the right index finger. This condition exposed an employee to a laceration and amputation hazard.    Abatement Note: Among other methods, one feasible and acceptable method to correct this hazard is to develop and implement specific procedural elements to control hazardous energy in accordance with the American National Standards Institute (ANSI)-American Society of Safety Engineers (ASSE) Z244.1-2003 publication.
