29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):    C.S. Roofing, at 2823 East Colfax Avenue, Denver, CO: On 9/27/11, C.S. Roofing did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  C.S. Roofing was replacing a three story tall building's roof.  C.S. Roofing did not perform an inspection of the worksite and equipment to ensure that employees were not exposed to a hazardous work condition.  This condition exposed employees to a fall hazard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure:    C.S. Roofing, at 2823 East Colfax Avenue, Denver, CO: On 9/27/11, C.S. Roofing did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure.  C.S. Roofing was replacing a three story tall building's roof.  Approximate height was thirty feet above ground level.  Employees were not protected by a guardrail system, safety net system, or personal fall arrest system.  This condition exposed two employees to an approximate thirty foot fall hazard.

Roofing, at 2823 East Colfax Avenue, Denver, CO: On 9/27/11, C.S. Roofing did not ensure that portable ladders used for access to an upper landing surface and the ladder side rails extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  C.S. Roofing was replacing a three story tall building's roof.  Employees used an aluminum ladder to access the first floor roof.  The ladder side rails extended approximately 1.5 feet above the upper landing surface.  The employer did not ensure the ladder side rails extended at least 3 feet above the upper landing surface.  This condition exposed two employees to an approximate ten foot fall hazard.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:    C.S. Roofing, at 2823 East Colfax Avenue, Denver, CO: On 9/27/11, C.S. Roofing did not ensure that ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.  C.S. Roofing was replacing a three story tall building's roof.  Employees used an aluminum ladder to access the first floor roof.  The ladder was located adjacent to the truck into which they dumped waste shingles and tar paper.  The employer did not ensure the ladder was secured to prevent accidental displacement.  This condition exposed two employees to an approximate ten foot fall hazard.
