29 CFR 1910.178(l)(2)(ii):     Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    Vernon Kuhns dba Kuhns Construction had employees operating rough terrain forklifts without first receiving the required elements of forklift operator training.  The employer did not provide practical training or an evaluation of the operators performance in the workplace.  All required forklift training elements include the following:    a.	Formal instruction (lecture, discussion, interactive computer learning, video tape, written material, etc.),  b.	Practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and  c.	Evaluation of the operators performance in the workplace.

The training shall include the following areas, as applicable:    Vernon Kuhns dba Kuhns Construction had an employee exposed to fall hazards in excess of 10 ft. while working from a supported scaffold without fall protection, on a construction site in Ridge Farm, Illinois.   The employer did not provide scaffold training.  This training would include:    a.	Nature of any electrical hazards, fall hazards, and falling object hazards in the work area,  b.	Correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems used,  c.	Proper use of the scaffold, and the proper handling of material on the scaffold,   d.	The maximum intended load and the load-carrying capacities of the scaffold used,  e.	Any other pertinent requirements of this subpart, L  Scaffolds.
