29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    On or about October 17, 2018, employees using chainsaws to cut frame posts were exposed to laceration hazards and fall hazards of approximately 26 feet and the employer did not ensure that employees were trained and able to recognize the hazards.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:    On or about October 17, 2018, employees using chainsaws to cut frame posts were exposed to laceration hazards and the employer did not ensure that employees were personal protective equipment to protect their hands and legs.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a. On or about October 17, 2018, an employee performing framing activities while working from the truss on the south end of the building to cut a vertical post was exposed to fall hazards of approximately 26 feet and fall protection was not used.    b. On or about October 17, 2018, an employee performing framing activities while working from the top plate of the northeast vestibule to cut a vertical post was exposed to fall hazards of approximately 9 feet and fall protection was not used.
