29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems: a) Kaminski Roofing, Inc., on or about October 4, 2011, at the address of 450 Thomas Alva Drive, Vermillion, OH: The employer did not assure employees were protected from fall hazards while applying asphalt shingles to a residential roof. The employees were exposed up to a 10 foot fall to the ground below.
