29 CFR 1910.307(c): Electrical installations. Equipment, wiring methods, and installations of equipment in hazardous (classified) locations shall be intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location. Requirements for each of these options are as follows: a) In the elevator, the junction box for the globe light had an opening, negating its approved for a class two(II) location. b) In the tripler area, the globe was missing from the globe light negating its approved for class two(II) loaction.

29 CFR 1910.110(h)(6)(ii)(b): Containers shall be protected by crash rails or guards to prevent physical damage unless they are so protected by virtue of their location. Vehicles shall not be serviced within 10 feet of containers. a) Propane fueling tank located outside of the maintenance shop adjacent to a designated employee parking lot was not protected by rails or guards to prevent it being struck or damaged by vehicular traffic.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated. a) Bin inspection drop light cord was not inspected before inserting into the bins, in that the grounding prong showed scorching.
