29 CFR 1910.23(c)(1):  Every open-sided floor or platform four feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides:    a.     There was no guardrail on the northeast side of the dryer platform.  Employees accessed the platform to set-up and run the dryer for the grain process, exposing employees to a fall hazard.    b.      There was no guardrail on the northwest side of the dryer platform.  Employees accessed the platform to set-up and run the dryer for the grain process, exposing employees to a fall hazard.    c.       There was no guardrail system on the south end of the dryer platform.  Employees accessed the platform to clean the dryer for the grain process, exposing employees to a seven foot, five inch fall hazard.

29 CFR 1910.23(a)(2):  Every ladderway floor opening or platform shall be guarded by a standard railing with standard toeboard on all exposed sides (except at entrance to opening), with the passage through the railing either provided with a swinging gate or so offset that a person cannot walk directly into the opening:    a.     On the top floor of the old side silo, employees use a fixed ladder to access the top of the silo.  The ladder-way access was not provided with a guardrail system and gate, or so offset that a person could not walk directly onto the opening, exposing employees to a six foot, three inch fall hazard.

29 CFR 1910.23(a)(9):  Every floor hole into which persons can accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than one inch wide. The cover shall be securely held in place to prevent tools or materials from falling through:    a.   On the first floor of the new side silo, the cover for a floor hole into which employees could accidently walk was not secured from displacement, exposing employees to a trip/fall hazard.

The guard shall have openings no larger than one-half  inch:    a.     On top of the new side silo, the Rolfes ventilation fan was not guarded against accidental contact, exposing employees to a contact/stuck-by hazard.

29 CFR 1910.219(d)(1):  Guarding. Pulleys, any parts of which are seven feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six foot, six inches from the floor or platform may be guarded with a disk covering the spokes:    a.    In the old side basement, the belt and pulley to the wet screw machine was not properly guarded, exposing employees to a caught-in hazard.    b.    In the old side electrical room, there was a guard missing from the back of the belt and pulley on the air compressor, exposing employees to a contact hazard.

29 CFR 1910.305(e)(1):  Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures. Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations shall be installed so as to prevent moisture or water from entering and accumulating within the enclosures and shall be mounted so there is at least 6.35-mm (0.25-in.) airspace between the enclosure and the wall or other supporting surface. However, nonmetallic enclosures may be installed without the airspace on a concrete, masonry, tile, or similar surface. The enclosures shall be weatherproof in wet locations:    a.    In the main electrical room, the employer had a 240 volt energized electrical outlet box which supplied power to equipment outside during harvest.  The box was not sealed or approved for outside use in damp or wet locations, exposing employees to a contact hazard.

29 CFR 1910.305(b)(1)(ii):  Unused openings in cabinets, boxes, and fittings shall be effectively closed:    a.     In the main electrical room, the pushmatic electrical center panel box had an unused opening in the bottom of the 240 volt energized panel, exposing employees to a contact hazard.    b.     In the main electrical room, the electrical disconnect for the manlift had an unused opening in the bottom of the energized disconnect, exposing employees to a contact hazard.    The Keynes Brothers Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.303(b)(7)(i), which was contained in OSHA inspection number 424536, Citation 1, Item number 6, and was affirmed as a final order on July 11, 2012, with respect to a workplace located at 901 South Court Street, Circleville, Ohio 43113.
