29 CFR 1910.178(m)(5)(i): When powered industrial trucks were left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, and brakes were not set (a) At the warehouse loading/unloading area, the Komatsu Forklift (Model No., FG18ST-17) was parked and left unattended, with the load engaging means in the raised position and parking brakes disengaged, thereby resulting in potential struck-by and caught-between hazards.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates or markings were not maintained in a legible condition: (a) At the main store, adjacent to the electronics area, the nameplate or marking of the "Crown" forklift was not maintained in a legible condition. The nameplate (also called the data plate) provides important information for the forklift operator, including the fuel type, forklift weight, and capacity. Operators should read the nameplate to know the forklifts capabilities and limitations.
