29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails,and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. Employees were exposed to scrap lumber with protruding nails from a 2-storey construction project at the Matafao Elementary School. The scrap materials were spread along the main path between the side of the new construction building and the beach.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Employees did not have fall protection while installing the 4th roof truss for a 2-storey constrction project at the Matafao Elementary School. The employee was straddling over the roof truss which was measured at 10 feet from the 2nd floor.
