29 CFR 1926.404(a)(2): A grounded conductor was attached to a terminal or lead so as to reverse designated polarity: a) Leone High School Project A, north side: On or about 09 MAY 12, the main outlet box for the electrical cords that provide current to the power tools such as the portable Skil Saw had reverse polarity exposing workers to an electrical shock hazard.
