29 CFR 1910.23(c)(1)(i):     Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall beguarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance toa ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides, persons can pass:    a.  HVAC Area of Electrical Shop - There was no standard railing including a toprail, midrail and toeboard provided for an open sided mezzanine storage area more than 12 feet above the concrete floor below potentially exposing employees to bodily injury.    b.  Plumbing Shop - The mezzanine storage area, which was more than 14 feet above the concrete floor below, did not have a toeboard potentially exposing employees who passed below to bodily injury.    c.  Electrical Shop - A mezzanine storage area, which was more than 12 feet above the concrete floor below, did not have a toeboard potentially exposing employees who passed below to bodily injury.

29 CFR 1910.215(b)(9):  The distance between the abrasive wheel periphery and the adjustable tongue or the end of theperipheral member at the top shall never exceed one-fourth inc:h:      a.  Wood Working Shop - The DeWalt bench grinder, Model No. DW758, S/N 200223-YI 241, had a tongue guard that was one half inch away from the edge of the abrasive wheel potentially exposing employees to the hazard of bodily injury.     Cycle Maintenance Shop - The Dayton 7 inch bench grinder Lot No. 200204, Model No. 4Z908C, had a tongue guard that was three eighths of an    inch away from the edge of the abrasive wheel potentially exposing employees to the hazard of bodily injury.
