29 CFR 1926.651(l): Guardrails complying with 1926.502(b) were not provided where walkways were 6 feet (1.8m) or more above lower levels:  (a) Construction site at 800 Highway 27 North, Augusta, WI; The employer constructed walkway did not meet the requirements of 29 CFR 1910.502(b), and had the following deficiencies;  A railing was missing on one side, the midrail was missing on the railing that was constructed, and there was greater than 19" between uprights on the railing that was constructed.
