29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: At the project on or about October 20, 2011, and at times prior thereto, there was no ground-fault circuit-interrupter protection or an assured equipment grounding conductor program when extension cords were used to power a 110 VAC Hilti drill. The drill was used to screw gypsum board onto metal studs.

29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers. When metal covers are used, they were not grounded. In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass were not provided with bushings designed for the purpose or they did not have smooth, well-rounded surfaces on which the cords may bear: At the project on or about October 20, 2011, and at times prior thereto, there was no cover plate on the outlet that powered extension cords used to power a 110 VAC Hilti drill.
