29 CFR 1910.146(c)(5)(ii)(C):      Before an employee enters the space, the internal atmosphere shall be tested, with a calibrated direct-reading instrument, for oxygen content, for flammable gases and vapors, and for potential toxic air contaminants, in that order. Any employee who enters the space, or that employee's authorized representative, shall be provided an opportunity to observe the pre-entry testing required by this paragraph.    a) On or about July 21, 2011 space was not tested before entrance to determine the levels within the bin in that employees were entering tanks that had rotting material present which depletes oxygen.

Equipment, wiring methods, and installations of equipment in hazardous (classified) locations shall be intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location. Requirements for each of these options are as follows:      a) The bin inspection droplight was not intrinsically safe, negating its approved for a class two(II) location.  b) The electrical disconect box in the basement was not intrinsically safe, negating its approved for a class two(II) location.  c) The electrical disconect box in the basement had two receptical that was not intrinsically safe, in that the recepticals were not encloed, negating its aproved for a class(II) location

29 CFR 1910.334(a)(2)(i):     Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.      a) Bin inspection drop light cord was not inspected before inserting into the bins, in that the grounding prong was mising.
