The intermediate rail was not approximately halfway between the top rail and the floor, platform, runway, or ramp: Employees working on the top of the bin deck outside the head house were exposed to fall hazards because the guardrail system was not constructed to provide adequate protection. The midrail was missing on a portion of the guardrail.
