29 CFR 1926.651(c)(2):     A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    On or about 9/25/11 during excavation activities, the employer did not ensure a safe means of egress was used when exiting a trench that measured 25 feet in length, 2 feet in width at the bottom of the trench, and ranged between 4 and 5.5 feet in depth.  The unprotected trench did not exceed 4.7 feet but the depth including concrete that was on top of the soil measured 5.5 feet in depth.          H.M. MILLER CONSTRUCTION CO.  WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD CFR 1926.651(C) (2), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER  312545049, CITATION NUMBER 01, ITEM NUMBER 01 AND WAS AFFIRMED AS A FINAL ORDER ON 11/06/08, WITH RESPECT TO A WORKPLACE LOCATED AT 4182 STATE RT. 43 KENT, OHIO 44240.    H.M. MILLER CONSTRUCTION CO.  WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD CFR 1926.651(C) (2), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315110940, CITATION NUMBER 01, ITEM NUMBER 01 AND WAS AFFIRMED AS A FINAL ORDER ON 12/29/10, WITH RESPECT TO A WORKPLACE LOCATED AT 1751 BRITTAN RD. AKRON, OHIO 44260.
