The Human Resources Manager signed the summary log.     b) The 300A Summary Logs for calendar year 2009 was not certified by the highest management official for the company. The Human Resources Manager signed the summary log.    c) The 300A Summary Logs for calendar year 2010 was not certified by the highest management official for the company. 29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.    The company's lock-out/tag-out procedures did not cover all forms of potentially hazardous energy when employees were performing maintenance and/or cleaning operations on the splitter machine.  The procedure left out the air valve to be locked and tagged out.    No Abatement Certification or Abatement Documentation Required.
