29 CFR 1910.36(g)(2): An exit access was not at least 28 inches (71.1 cm) wide at all points and the width of the exit and exit discharge was not at least equal to the width of the exit access:    a) On or about May 12, 2013 an employee was blocked between stacks of trays and the main conveyor line while stacking off bread.     b) On or about June 24, 2013 the egress routes were obstructed for line employees working between and at the end of lines 1 and 2 in the packaging department.     Abatement Certification and Abatement Documentation Required.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points, moving parts and caught in hazards:      a)    On or about May 22, 2013 an in-running nip point created by Slicer No. 1 conveyor and the in-feed roller transition plate was not lowered to guard against a caught-by and pinch hazards.     b) On or about May 2, 2013 the unused portion of the blades on the reciprocating slicer were not guarded to prevent employee exposure to rotating blades.       Abatement Certification and Abatement Documentation Required.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:  a) On or about June 24, 2013 a vertical plexiglass shield used on the reciprocating slicer near Slicer No. 1 line, was attached to the slicer using various thumb-knob hardware and did not require any tooling to remove the shield.     b) On or about June 24, 2013 a horizontal plexiglass shield used to cover the out-feed portion of the reciprocating slicer and to keep employee(s) hands away from the reciprocating blades, was not secured to the slicer.   c) On or about June 24, 2013 the metal guards used on the bottom portion of the bagger machine used at the end of Slicer No. 1 were not secured to the bagger in a way that required any tooling to remove.
