29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:    (a) At the backside of the International Sports Center, a 7-1/4 Skilsaw, model 5155, was not maintained free from recognized hazards. There was damage to the cords insulation, exposing energized conductors. Employees were exposed to electrical shock.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.    (a) At the project site, spoil material was not moved back at least 2 feet from the edge of excavation. Employees were exposed to potential cave-in hazards associated with the weight of the spoil, causing serious injuries.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) At the rear of the building, a four foot step ladder did not extend at least three feet above the roof, exposing employees that used the ladder to the hazard of falling from a height of about 4 feet to the ground below.
