On November 1, 2011 at a jobsite located at 990 County Farm Road, Carol Stream IL. An employee was working from a scaffold with no means of fall protection. The employee was exposed to a fall over 25 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
