29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    On November 1, 2011 at a jobsite located at 990 County Farm Road, Carol Stream IL. An employee was working from a scaffold with no means of fall protection. The employee was exposed to a fall over 25 feet.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On November 1, 2011 at a jobsite located at 990 County Farm Road, Carol Stream IL. An employee was working on a roof without utilizing any means of fall protection. The employee was exposed to a fall hazard over 25 feet.
