29 CFR 1926.1053(b)(1):     Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder���s length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.      On July 11, 2012 employees were using an extension ladder that did not extend three feet past the roof they were accessing, exposing then to a fall hazard of over 25 feet.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

Bryja Remodeling, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in inspection number 313621807, citation number 1, item number 2, and was affirmed as a final order on August 18, 2009, with respect to a work place located at 1617 Lake Ave. Wilmette, IL. 60091.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
