29 CFR 1910.23(c)(1):     Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance toa ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides, persons can pass.    I.H. Schlezinger - 1041 Joyce Avenue, Columbus Ohio worksite:     a.) As of and prior to November 3, 2011, for a five foot section located at the east end of the open sided "Picking" platformon the north side of the area,  the linked steel chain that was installed as standard railing was inadequate, in that the three lengths of chain intended to be equivalent to standard railing, had significant "deflection." The top section, or top rail equivalent, had an eleven inch deflection from the forty two inch height at the installation point on the upright posts. The two sections installed to be equivalent to midrail individually had seven inch and twelve inch deflections from the twenty eight inch and fourteen inch installation points on the upright posts. This condition exposes employees performing "picking" tasks to an eight foot fall hazard.    b.) As of and prior to November 3, 2011, for a three foot section located at the ladder end of the open sided "Picking" platform on the north side of the area,  the linked steel chain that was installed as standard railing was inadequate, in that the three lengths of chain intended to be equivalent to standard railing, had significant "deflection." The top section, or top rail equivalent, had a ten inch deflection from the forty two inch height at the installation point on the upright posts. The two sections installed to be equivalent to midrail individually had seven inch and six inch deflections from the twenty eight inch and fourteen inch installation points on the upright posts. This condition exposes employees performing "picking" tasks to an eight foot fall hazard.      c.) As of and prior to November 3, 2011, for a section of open sided floor located near the south side ladder, the linked steel chain that was installed as standard railing was inadequate, in that the three lengths of chain intended to be equivalent to standard railing, had significant "deflection." The top section, or top rail equivalent, had a five inch deflection from the forty two inch height at the installation point on the upright posts. The two sections installed to be equivalent to midrail individually five seven inch and four inch deflections from the twenty eight inch and fourteen inch installation points on the upright posts. This condition exposes employees accessing the Shanty to an eight foot fall hazard.

Vertical clearance above any stair tread to an overhead obstruction shall be at least 7 feet measured from the leading edge of the tread.[39 FR 23502, June 27, 1974, as amended at 43 FR 49744, Oct. 24, 1978; 49 FR 5321, Feb. 10, 1984]    I.H. Schlezinger - 1041 Joyce Avenue, Columbus Ohio worksite: As of and prior to November 1, 2011, the fixed industrial stairway for the receiving hopper platform did not have the required overhead clearance. There is electrical equipment (a run of 3 piped conduit) that is only 53 inches above the tread for the fourth riser and 44 inches above the tread for the fifth riser. Maintenance employees utilize the stair to access the receiving hopper to perform maintenance and repairs on equipment.

29 CFR 1910.24(h):     "Railings and handrails." Standard railings shall be provided on the open sides of all exposed stairways and stair platforms. Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails shall be installed in accordance with the provisions of 1910.23.    I.H. Schlezinger - 1041 Joyce Avenue, Columbus Ohio worksite:     a.) As of and prior to November 1, 2011, the fixed industrial stairway for the receiving hopper platform did not have standard railing provided on both open sides. The left hand side (ascending) did not have standard railing installed. Maintenance employees utilize the stair to access the receiving hopper to perform maintenance and repairs on equipment.    b.) As of and prior to November 1, 2011, the fixed industrial stairway platform for the receiving hopper did not have adequate standard railing provided on the back (open) side - opposite the stairway. A metal pallet is "installed" in lieu of standard railing that is held in place with "mechanics" wire or "baling" wire. Maintenance employees utilize the stair platform to access the receiving hopper to perform maintenance and repairs on equipment.    c.) As of and prior to November 1, 2011, the fixed industrial stairway on the north side of the picking station "shanty" did not have standard railing for the open side at the bottom of the stair. A concrete wall begins at the fifth riser making the bottom section of the platform open sided on the machinery side of the stair.    d.) As of and prior to November 3, 2011, the fixed industrial stairway on the south side of the picking station "shanty" did not have standard railing for the open side at the bottom of the stair.   A concrete wall begins at the fifth riser making the bottom section of the platform open sided on the machinery side of the stair.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means:    I.H. Schlezinger - 1041 Joyce Avenue, Columbus Ohio worksite: As of and prior to November 1, 2011, the live parts of electrical power supply equipment mounted at the base of the short stacker conveyor was not protected from damage. The metal conduit was damaged after having been pulled apart by the movement of the mechanical equipment associated with this process. Employees have exposure to this electrical contact hazard during daily cleanup activities.

29 CFR 1910.305(b)(2)(i):     All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    	    I.H. Schlezinger - 1041 Joyce Avenue, Columbus Ohio worksite: A junction box mounted on the Shear Operator booth did not have a cover installed. Due to this condition, employees have exposure to the exposed live parts and are at an increased risk for electrocution associated with this electrical contact hazard.

29 CFR 1910.147(c)(4)(ii)(C): Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them;     I.H. Schlezinger - 1041 Joyce Avenue, Columbus Oio worksite: As of and prior to November 3, 2011, specific lockout/tagout procedures for the Harris 6511 Shear were inadequate as the placement for the isolation device was incorrectly noted. The procedure listed the location as electrical disconnect boxes in the main electrical area in the baler break room. The actual location to be used for placement of lockout/tagout isolation devices for this procedure is the Shear building pump room.
