29 CFR 1926.200(g)(1): Construction areas were not posted with legible traffic signs at points of hazards: a) Eagle Road northbound, between Ustick and McMillan Roads; On or about November 2, 2011 and at times prior thereto, adequate advanced warning signs had not been placed to alert approaching vehicles about overhead utility work being conducted along side of the roadway. b) Eagle Road and W. Meadowdale Drive intersection; On or about November 2, 2011 and at times prior thereto, advanced warning signs had not been placed to alert approaching vehicles about a cable laying across the roadway. c) Eagle Road and W. Meadowdale Drive intersection; On or about November 2, 2011 and at times prior thereto, 'Flagger Ahead' signs had not been placed to alert approaching vehicles about a flagger being present on the roadway. Note: Abatement certification is NOT required for this item.

29 CFR 1926.201(a): Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers did not conform to Part VI of the Manual on Uniform Traffic Control Devices, (1988 Edition, Revision 3 or the Millennium Edition), a) Eagle Road and W. Meadowdale Drive intersection; On or about November 2, 2011 and at times prior thereto, an employee signaling traffic that was approaching/passing over a cable lying across the roadway was not wearing a high visibility garment. b) Eagle Road and W. Meadowdale Drive intersection; On or about November 2, 2011 and at times prior thereto, an employee signaling traffic that was approaching/passing over a cable lying across the roadway was not utilizing a stop/slow paddle or a flag. c) Eagle Road and W. Meadowdale Drive intersection; On or about November 2, 2011 and at times prior thereto, an employee signaling traffic that was approaching/passing over a cable lying across the roadway had not been provided with traffic control training and/or instructions. Note: Abatement certification is NOT required for this item.
