29 CFR 1926.100(a):     Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a)(LOCATION)(SPECIFIC OPERATION(S) AND/OR CONDITIONS)    (a)  At the 9704-9706 W. Lincoln Avenue, West Allis, Wisconsin jobsite, an employee was working on an extension ladder below residential roofing activities without wearing head protection. The employee was exposed to struck-by hazards.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    (a)  At the 9704-9706 W. Lincoln Avenue, West Allis, Wisconsin jobsite, an employee was performing residential roofing activities on an approximate 4/12 pitch roof without adequate fall protection. The employee was exposed to a 19.6 foot fall hazard.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) At the 9704-9706 W. Lincoln Avenue, West Allis, Wisconsin jobsite, the Werner extension ladder used to gain access to the upper roof landing was extended approximately 1 rung above the landing. Employees were exposed to approximately 19.6 foot fall hazards.
