29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a) On or about October 28, 2014 at the 1006 E Montana Street, Milwaukee, Wisconsin jobsite, employees working on the ground and/or ladder below residential roofing activities were not wearing protective helmets. Tri-County Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1926.100(a)], which was contained in OSHA inspection number 108546, citation number 1, item number 1 and was affirmed as a final order on December 28, 2011, with respect to a workplace located at 9704-9706 W Lincoln Avenue, West Allis, Wisconsin 53227.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about October 28, 2014 at the 1006 E Montana Street, Milwaukee, Wisconsin jobsite, the employer did not ensure that all employees performing residential roofing activities were protected from falling approximately 20 feet to the lower level. Tri-County Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1926.501(b)(10)], which was contained in OSHA inspection number 943526, citation number 1, item number 1 and was affirmed as a final order on January 14, 2014, with respect to a workplace located at 8600 Green Bay Road, Pleasant Prairie, Wisconsin 53158. Tri-County Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1926.503(b)(13)], which was contained in OSHA inspection number 911927, citation number 1, item number 1 and was affirmed as a final order on August 19, 2013, with respect to a workplace located at 6285 S Hillendale Drive, New Berlin, Wisconsin 53151.
