29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY) (a) Employees working on a low slope roof were not protected from falling to the lower level. This exposed employees to a fall hazard in excess of 40 feet. The Tri-County Contracting, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 911927.015, citation number 1, item number 1, and was affirmed as a final order on August 19th, 2013, with respect to a workplace located at 6285 Hillendale Dr., New Berlin, WI 53146.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) The portable ladder being used to access the low slope roof did not extend at least three feet above the upper landing surface. The Tri-County contracting, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 108546.015, citation number 1, item number 3, and was affirmed as a final order on December 28th, 2011, with respect to a workplace located at 9704-9706 W. Lincoln Ave., West Allis, WI 53227.
