29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    (a) At lot number 5172-1-7-3-2, an employee entered the construction site without protective foorwear thus, potentially exposed to hazards of falling and rolling objects, cuts and punctures.

29 CFR 1926.452(a)(2): Crossbracing was not installed between the inner and outer sets of poles on double pole scaffolds:     At lot number 5172-1-7-3-2, the double pole scaffold did not have crossbracing between every set of outer and inner poles, exposing employees to the hazard of falling from a height of about 9.5 feet from the platform to the ground.

At Lot 5172-1-7-3-2, the double pole scaffold did not have diagonal bracing in both directions installed across the entire outside face, exposing employees to the hazard of falling from a height of 9.5 feet from the platform to the ground.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:    (a) At lot number 5172-1-7-3-2, an employee was using the interior stairway to access the second floor. The stairway was not equipped with at least one handrail and one stairrail system along each unprotected side or edge thus, potentially resulting in fall to the lower level.
