29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) On or about October 26th, 2011 an employee attempted to clean a Translicer 2500 Cutter he was operating. The employee lifted the metal cover enclosing the point of operation.  When the employee reached into the machine, he came into contact with the rotating cutting blades that were still coasting to a stop after the electrical interlock switch had been disconnected. The employee suffered amputation injuries to two fingers as a result.     b) The company did not have a lockout/tagout program that met all of the requirements of this standard including, but not limited to a written program and procedures, employee training, and periodic inspections.
