29 CFR 1926.1053(b)(1):   Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a)  At the 1025-1027 S. Layton Blvd., Milwaukee jobsite; several extension ladders, used to access the residential roof, did not extend at least three feet past the landing exposing employees to slipping and fall hazards.

29 CFR 1926.451(g)(1)(i):   Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  (a)  At the 1025-1027 S. Layton Blvd., Milwaukee jobsite; fall protection was not utilized while working from a ladder jack scaffold approximately 19.4' above ground performing roofing activities.  This exposed employees to fall hazards.

29 CFR 1926.501(b)(13):   Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.  (a)  At the 1025-1027 S. Layton Blvd., Milwaukee jobsite; adequate fall protection was not utilized while working from a residential roof approximately 22.1' above ground exposing employees to fall hazards.
