29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: (a) Near the west side of the house on or about November 3, 2011, and at times prior thereto, debris was not kept cleared from the path to the entries to the house. There was chunks of CMUs, CMUs, 2x4s, and other lumber in the path.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that will prevent pull from being directly transmitted to joints or terminal screws: (a) In the garage attached to the house on or about November 3, 2011, and at times prior thereto, an extension cord's outlet was not secured to the outside insulation on the extension cord. The cord was used to power a 110 VAC Makita circular saw.
