At the site the employer did not ensure that all guardrails were installed along all open sides of the scaffold while employees were working from it. Armando Hernandez was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.451(g)(4)(i), which was contained in OSHA inspection number 310479480, citation number 1, item number 3 and was affirmed as a final order on April 30, 2008, with respect to a workplace located at Bldg 50 Cross River Falls Blvd, Dublin, Ohio 43016.
