29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift:    (a) In front of the building on or about November 7, 2011, the lanyard was not properly attached to the aerial lift basket as it was attached to the basket's guardrails instead of the tie off ring near the basket platform as instructed by a manufacturer's label on the lift.  The aerial lift, model number MHP15/44, serial number N2080436, was made by Snorkel in New Zealand in April 2008.  An employee painted the exterior of the building from the lift's basket about 21 feet above the concrete below.
