29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    (a) At the project site, head protection was not used while employees were erecting a mobile scaffold. Employees were exposed to the hazards of loose scaffold components and small hand tools striking their heads.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    At the project site at front of Unit 101, ladders or stairs were not used to access completed scaffold platforms.  Employee climbed the end frame of the mobile scaffolds to access the platforms. Employee was exposed to the hazard of falling from heights that varied from about three feet to 10 feet.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:    (a) At the project site in front of Unit 101, the mobile scaffold was accessed without locking the wheels.
