29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions andthe regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. Trucano Landing, Supply Barge "Foss 106": On or about November 3, 2011 and at time prior thereto, the employer did not provide formal training regarding the hazards associated with the loading and unloading of material barges afloat. This condition directly impacted the stability of the platform in which a P&H Model 535, Serial Number 30336 crawler crane operated. The instability created by the situation resulted in fatal injuries to the operator of the crane.

29 CFR 1904.39(b)(1): If the Area Office is closed, may I report the incident by leaving a message on OSHA's answering machine, faxing the area office, or sending an e-mail? No, if you can't talk to a person at the Area Office, you must report the fatality or multiple hospitalization incident using the 800 number: Trucano Landing: On or about November 3, 2011, a voicemail report of an occupational fatality at the employer's worksite was reported to an unattended phone extension at the Juneau office of the State of Alaska, Department of Labor and Workforce Development, Occupational Safety and Health. This condition delayed the prompt response of Anchorage Area Office compliance personnel to conduct an immediate and thorough investigation of the incident.
