29 CFR 1910.134(f)(2):     The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter.      Fiberglass Fabrication CONX:  On or about November 3, 2011 and at times prior thereto, the employer could not produce documentation that the employee had been fit tested with a North halfmask, negative-pressure, air-purifying respirator annually.  This condition exposed employees to inhalation hazards in the event the chosen respirator was not properly fitted.

29 CFR 1926.303(c)(5):     When safety guards are required, they shall be so mounted as to maintain proper alignment with thewheel, and the guard and its fastenings shall be of sufficient strength to retain fragments of the wheel in case of accidental breakage. The maximumangular exposure of the grinding wheel periphery and sides shall not exceed 180 deg.      Fiberglass Fabrication CONX:  On or about November 3, 2011 and at times prior thereto, a Dewalt Model Number D28402 four and one-half (4.5) inch portable grinder was not used with the manufacturer's safety guard.  This condition exposed employees to struck-by hazards.
