Dietary workers used Ecolab Inc San Max (containing didecyldimethylammonium chloride) to clean surfaces in the kitchen. The employer did not ensure that employees wore eye protection when using this liquid chemical. This condition exposed employees to a chemical hazard. Balfour Cherrywood Village LLC, at 282 McCaslin Boulevard, Louisivlle, CO: On and preceding 11/8/11, Balfour Cherrywood Village LLC did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from liquid chemicals, acids or caustic liquids. Housekeeping staff dispensed Diversey Oxivir TB (containing 0.1 -1.5% hydrogen peroxide) from primary containers into spray bottles using a hose. 29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: Balfour Cherrywood Village LLC, at 282 McCaslin Boulevard, Louisivlle, CO: On 11/8/11, Balfour Cherrywood Village LLC did not ensure that where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were provided within the work area for immediate emergency use. Dietary workers used The Clorox Company Regular Clorox Bleach (containing 5.25% sodium hypochlorite) to clean surfaces and Reckit Benckiser Professional Easy-Off Heavy Duty Oven & Grill Cleaner (containing 4-6% sodium hydroxide) to clean the oven. Sodium hypochlorite and sodium hydroxide are corrosive liquids. The employer did not provide an emergency eyewash in the kitchen. This condition exposed employees to a chemical burn hazard.

Nurses used One Touch UltraSoft pen-type lancets when caring for diabetic residents. The lancet design required removal of the contaminated lancet tip following use. The pen-type lancets exposed nurses to contaminated sharps following use and during disposal of the contaminated sharps. The employer did not utilize engineering controls that eliminated or minimized employee exposure. This condition exposed employees to a bloodborne pathogen hazard.

Caregivers used disposable razors to shave residents of a long term assisted living facility. The employees recapped the contaminated sharps prior to disposal into trashcans. The employees then transported the trashcans to the biohazard waste can to dispose the razor. The razor was not placed into a sharps container. This condition exposed employees to a bloodborne pathogen hazard. Balfour Cherrywood Village LLC, at 282 McCaslin Boulevard, Louisivlle, CO: On and preceding 11/8/11, Balfour Cherrywood Village LLC did not ensure that during use, containers for contaminated sharps were easily accessible to personnel or located as close as was feasible to the immediate area where sharps were used or could be reasonably anticipated to be found. Nurses used One Touch UltraSoft lancets and BD SafetyGlide insulin syringes when caring for residents of a long term assisted living facility. The employees carried the contaminated sharps approximately twenty feet from the interior of the resident's room to the nearest sharps container, located across the hall from the resident room, locked within a wall cabinet. This condition exposed employees to a bloodborne pathogen hazard.
