29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage: (a)Balfour Cherrywood Village LLC, at 282 McCaslin Boulevard, Louisivlle, CO: On and before 12/4/14, Balfour Cherrywood Village LLC did not protect flexible cords and cables from accidental damage. The two flexible cords supplying 120 volt power to two Carter Hoffmann "hotboxes" model BB96-85 in the kitchen were damaged. The conductors for each power cord were exposed. This condition exposed employees to an electrical hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a)Balfour Cherrywood Village LLC, at 282 McCaslin Boulevard, Louisivlle, CO: On and before 12/4/14, Balfour Cherrywood Village LLC did not ensure that flexible cords were connected to devices and fittings so that tension would not be transmitted to joints or terminal screws. Two flexible cords supplied 120 volt power to two Carter Hoffmann "hotboxes" model BB96-85 in the kitchen. Strain relief had not been provided to either power cord at the fitting where each cord entered the hotbox or at the receptacle that each cord was plugged into. This condition exposed employees to an electrical hazard.

Nurses used Ultimed Inc UltiCare tuberculin syringes and Retractable Technologies VanishPoint syringes during healthcare duties at the assisted living facility. The bloodborne pathogens training program did not include an explanation of the use and limitations of medical devices used by nurses. This condition exposed employees to a bloodborne pathogen hazard. Balfour Cherrywood Village LLC, was previously cited for a violation of this Occupational Safety and Health Administration Standard or its equivalent standard, 29 CFR 1910.1030(g)(2)(vii)(F), which was contained in OSHA inspection 108781, Citation 1, Item 4 and was issued on December 9, 2011, and affirmed as a final order on January 3, 2012, with respect to a workplace located at 282 McCaslin Boulevard, Louisville, CO.

29 CFR 1910.1030(c)(1)(iv)(B): The annual review and update of the Exposure Control Plan did not include annual consideration and implementation of appropriate commercially available and effective safer medical devices designed to eliminate or minimize occupational exposure: (a)Balfour Cherrywood Village LLC, at 282 McCaslin Boulevard, Louisivlle, CO: On and preceding 12/4/14, Balfour Cherrywood Village LLC did not ensure that the annual review and update of the Exposure Control Plan included annual consideration and implementation of appropriate commercially available and effective safer medical devices designed to eliminate or minimize occupational exposure. Nurses, caregivers, and housekeepers cared for residents of a long term assisted living facility. Employees were occupationally exposed to blood or other potentially infectious materials (OPIM). Nurses used Ultimed Inc UltiCare tuberculin syringes and Retractable Technologies VanishPoint syringes during healthcare duties at the assisted living facility. The review of the Exposure Control Plan did not include review of medical devices, including syringes, used by nurses. This condition potentially exposed employees to a bloodborne pathogen hazard.
