29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Durabilt Products- On or about 11/08/11, the employer did not establish an energy control (lockout/tagout) program which includes energy control procedures, employee training and periodic inspections to ensure employees performing servicing or maintenance on machinery or equipment. Employees were thereby exposed to the hazards associated with unexpcected energization of metal and glass abrasive blasting machines and electrical wiring within the facility.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.304(a)(2):     Polarity of connections. No grounded conductor may be attached to any terminal or lead so as to reverse designated  polarity.    a) Lathe Table- On or about 11/08/11, the four outlet electrical box that provided energy to an adjustable light had reverse polarity exposing employees to electrical hazards.    b) Check in Area- On or about 11/08/11, the four outlet electrical box that provided energy to a portable light had reverse polarity exposing employees to electrical hazards.    c) Work Bench- On or about 11/08/11, the four outlet electrical box that provided energy to a overhead shop light had reverse polarity exposing employees to electrical hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

Rear exit doors- On or about 11/08/11, rear exit doors did not have a sign indicating from the inside that they were exits.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.37(b)(5):     Each doorway or passage along an exit access that could be mistaken for an exit must be marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g., closet).    Rear doors- On or about 11/08/11, three doors located at the rear of the buildiing did not have the purpose marked on the door or the designation for the door being an exit. Employees were exposed to the hazard of not being able to exit the building in case of emergency.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.178(l)(1)(i):     The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a) Durabilt Products- On or about 11/08/11, employees operating a forklift had not been trained and evaluated to ensure competency in the use of equipment.
