29 CFR 1910.119(j)(3): Training for process maintenance activities. The employer shall train each employee involved in maintaining the on-going integrity of process equipment in an overview of that process and its hazards and in the procedures applicable to the employee's job tasks to assure that the employee can perform the job tasks in a safe manner. The employer did not adequately train maintenance helpers assigned to work in the UDEL unit and as a result a piece of critical equipment was not adequately assembled.

29 CFR 1910.119(j)(4)(ii): Inspection and testing procedures shall follow recognized and generally accepted good engineering practices. In the UDEL unit, the employer did not perform an adequate inspection of the PE-306 heat exchanger to detect possible leakage and did not adequately test the equipment before returning it to service.

29 CFR 1910.119(l)(1): The employer shall establish and implement written procedures to manage changes (except for "replacements in kind") to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process. In the UDEL unit, the employer had a written procedure to manage changes to the process, however the employer did not manage the change to the C-304 PSV device, which had a flange that was designed for eight bolts but had only four inserted during operation of the PC-304 reactor.
