Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the fall protection system utilized incompatible components:     (a) ABCO Contracting, Inc. @ 1st Avenue and Downing Street, Denver, CO 80218: On and before 11/10/11, the employer equipped employees with flexible, elastic-style, energy absorbing 60 inch lanyards to be used in conjunction with Protecta Rebel Self-Retracting Lifelines (SRL). The Rebel SRL was also equipped with a 24 inch energy absorbing pack. The use of a lanyard in conjuction with the Rebel SRL is not approved by the manufacturer. This condition exposed employees to a fall hazard ranging from 12-14 feet.

29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two:    (a) ABCO Contracting, Inc. @ 1st Avenue and Downing Street, Denver, CO 80218: On and before 11/10/11, the employer did not ensure that the horizontal lifeline system, which was composed of a continuous strand of 1/2 inch cable and connected to type 7 safety barriers, was designed and maintained by a qualified person. This condition exposed employees to a fall hazard of approximately 12-14ft.

(b) ABCO Contracting, Inc. @ 1st Avenue and Downing Street, Denver, CO 80218: On and before 11/10/11, employees were exposed to sound levels in excess of the referenced levels on Table D-2 of the same paragraph. Employee 2 performed bridge deck demolition and was exposed to a dose of 147.4% or 92.7 dBA. This condition exposes employees to permanent hearing damage.
