29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection measure provided. (a) M&M Roofing @ 3087 Sanford Circle, Loveland, CO 80537: On and before October 1st, 2011, the exposing employer did not ensure that fall protection systems were utilized by an employee engaged in roofing activities while working at the unprotected edges of a roof on a single-family residential housing. Employees were exposed to fall hazards of approximately 12'.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder (a) M&M Roofing @ 3087 Sanford Circle, Loveland, CO 80537: On and before October 1st, 2011, the exposing employer did not ensure that employees were utilizing portable ladders in such a way that enabled them to access and egress from work areas using ladders that extended 3' above the walking working surface. Employees were using a portable extension-ladder for access to the roof. Employees were exposed to a fall of approximately 12' while attempting to use the ladder.
