CFR 1910.157(c)(1):  The employer failed to provide portable fire extinguishers and mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury    	(a)	American Window Insulating Company, Inc. 358 S. Navajo Street, Denver, CO 80223:  On and before November 16, 2011, the controlling and exposing employer failed to position portable fire extinguishers so that they were readily accessible to employees.  Specifically, three portable CO2 extinguishers were mounted on walls in the main welding area in locations where employees could not access them safely due to material being stored in front of them.  Condition exposed employees to fire hazards and lack of adequate fire fighting equipment.

(a)	American Window Insulating Company, Inc. 358 S. Navajo Street, Denver, CO 80223:  On and before November 16, 2011, the controlling and exposing employer failed to assure that portable fire extinguishers were maintained in a fully charged condition.  Specifically, a portable fire extinguisher located in the canvas cutting area was observed to have been discharged and was still mounted on the wall. Condition exposed employees to fire hazards and lack of adequate fire fighting equipment.

CFR 1910.157(d)(2):  The employer failed to distribute portable fire extinguishers for use by employees on Class A fires so that the travel distance for employees to any extinguisher was 75 feet (22.9 m) or less.     	(a)	American Window Insulating Company, Inc. 358 S. Navajo Street, Denver, CO 80223:  On and before November 16, 2011, the controlling and exposing employer failed to distribute portable fire extinguishers for use on class A fires so that the travel distance for employees to any extinguisher was 75 feet or less.  Specifically, only one portable CO2 extinguisher was mounted on the wall in the paint and powder coating area, and it was blocked and located near the entrance at a distance of well over 75 feet from two employee work stations at the far end of the paint and assembly area.  Condition exposed employees to fire hazards and lack of adequate fire fighting equipment.

Navajo Street, Denver, CO 80223:  On and before November 16, 2011, the controlling employer did not ensure that specific procedures for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy were developed for all of the machinery which required it on site, such as the main cutting saw and powder coating oven. This condition exposed employees to amputation and point of operation hazards while servicing machinery.
