OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to fall and crushing hazards:    On or about 10/7/11, employees riding a conveyor belt, approximately 13 feet 10 inches high at its highest point, near the opening of the baler, were exposed to hazards such as, but not limited to, fall hazards, due to accumulated cardboard and noncardboard materials on the belt; and crushing injuries, with increased proximity to the baler.
